IMPORTANT: EPA now requires that you purchase the Uniform Hazardous Waste Manifest from an approved source. REG ALERT Changes to hazardous waste rules in 40 CFR parts 260 through 271 require use of the revised Uniform Hazardous Waste Manifest and Continuation Sheet forms to track hazardous waste from a generator’s site to the site of its disposition.
Only the revised manifest form from an approved source is valid and acceptable for use, and all shipments of hazardous waste initiated by generators or offerors must include the revised form.
